Re-Enrollment Application Form

 
We are so happy that you're interested in re-enrolling at Bryn Mawr. We welcome your application. Please complete the following questions with care by providing thoughtful responses to the questions and complete the application in full by submitting all required supporting materials, such as official transcripts and letters of support from employers and/or instructors, to reenrollment@brynmawr.edu.

If you are on a health leave of absence (formerly a medical or psychological leave), do not submit copies of the Healthcare Provider Assessment Form to reenrollment@brynmawr.edu. Instead:
  • If you are addressing a physical condition or symptoms while on a health leave, please contact the Health Services Director, Beth Kotarski, to submit your Healthcare Provider Assessment Form and to discuss your care plan. 
  • If you are addressing a mental health condition or symptoms while on a health leave, please contact the Director of Counseling, Luci MacNamara, to submit your Healthcare Provider Form and to discuss your care plan. 
Make sure all required materials are submitted by the application deadline:
  • If you hope to return in the fall semester, please submit the application by March 1.
  • If you hope to return in the spring semester, please submit the application by October 15. 

If the Re-Enrollment Committee does not receive all the required materials by the deadline, they will be unable to review your application and your leave will be extended by an additional semester.
