Bryn Mawr Community COVID-19 Health and Safety Concern Form

Any member of the Bryn Mawr Community may complete this form to submit one of the following:
 
General Concerns
  • Please use this option to relay any general comments that you have regarding Bryn Mawr's COVID-19 response.
  • A copy of your submission will go directly to a member of Bryn Mawr's Operations Planning Group so that your comments may be addressed accordingly.
Incident Report
  • Please use this option to report on a specific incident of concern that you yourself have observed or have learned may occur in the future and that might put members of the community at risk. In the spirit of care and respect for our community, the College encourages community members to respectfully raise concerns with each other as issues arise. Whether the community member is able to intervene or not in a given circumstance, we ask that they report the incident so that the College can be aware and helpfully take action.
  • You will be asked to provide as much information as possible regarding the incident including the issue(s), location, date, time, and approximate number of individuals involved.
  • A copy of your submission will go directly to a member of Bryn Mawr's Operations Planning Group so that the incident may be recorded and actions taken to encourage health and safety compliance going forward.
 
This form should not be used in the event of a medical emergency or an incident occurring now. If you are concerned about the well-being of a Bryn Mawr community member or you are currently observing concerning behavior on campus, please call Campus Safety at 610-526-7911 immediately.
